Rotation Description:
The Department of Family Medicine delivers patient-centered care for patients of all ages. The Family Medicine Center is an ambulatory care site that has a pharmacist-led pharmacotherapy clinic that focuses on management of chronic diseases such as anticoagulation, heart failure, hypertension, diabetes, asthma, and chronic obstructive pulmonary disease. Additionally, pharmacists are integral team members in multidisciplinary transitions of care visits for patients who have recently been discharged from the hospital. Additional responsibilities include teaching Family Medicine residents, answering drug information questions for Family Medicine faculty and residents, writing and reviewing medication-related policies and procedures, contributing to various quality improvement initiatives, and providing recommendations, counseling, and education to patients as needed. Learners are expected to actively participate in patient care, drug information queries, and administrative responsibilities.
