Rotation Description:

The UNC Geriatrics Specialty Clinic provides comprehensive care to older adults and their families, providing a focus on maintaining independence, quality of life, family support and helping everyone live as well as they can for as long as they can. The pharmacist plays an integral role in the clinical services by evaluating the whole patient assessing all disease states and all medications, including prescription and non-prescription medications, alternative/complimentary medications, and dietary supplements. The pharmacist is integrally involved in the training of Geriatric fellows and works within the interdisciplinary team to assist the Geriatric fellow with optimizing the medication regimen to meet the patient's goals. The pharmacist also manages a geriatric anticoagulation clinic and medication management clinic working collaboratively with the Geriatric providers to optimize medications and reach drug therapy goals. The pharmacist provides these same services at a local continuing care retirement community. A multitude of other services are offered at this site, including opportunities to provide patient education, assist with medication access/affordability, and assist with strategies to improve adherence.
