UNC CareLink

Access UNC CareLink from any page on our website by clicking on the link at the top right. Our web-based referral portal provides online access these features:

  • A detailed overview of your patient's medical record
  • Secure messaging with UNC Health providers regarding your patient's care
  • Online referrals for outpatient and ancillary services
  • Access to scheduled appointments, medications, allergies, and outpatient & inpatient notes (including admissions, discharges and emergency services)
  • Helps with CMS Meaningful Use and patient-centered medical home efforts

UNC Carolina Consultation Center

The UNC Carolina Consultation Center (CCC) no longer supports consult requests. The CCC phone number remains operational to re-route calls:

  • Phone consultations for pediatric outpatient consults remain unchanged and will route to the UNC Med Telecom (Hospital Operator) team.
  • Consult request for mutual patients (i.e., patients with an established relationship with both the referring clinician and the requested specialty) will route to the UNC Med Telecom (Hospital Operator) team.
  • Inpatient transfer requests will route to the UNC Patient Logistics Center.

UNC CareLink allows clinicians to refer to UNC Health specialists, as appropriate.* Adult outpatient requests can access CareLink or E-Consults, if available.

Please ensure any specialty service information is included on your patient-facing website (i.e., UNCHealth.org).

Note: UNC Health no longer offers phone-based consultations for adult services and pediatric Neurology.

UNC Heart & Vascular Open Access Physician Referral Service

Open Access is a physician referral service, created by the UNC Center for Heart & Vascular Care, which coordinates all admissions and transfers through a single phone call and guarantees immediate acceptance for patients.

UNC Center for Transplant Care Physician Referrals

At UNC Health, referring physicians are an integral part of our care team. Whether you’re collaborating with our medical team to help determine the best course of treatment for your patient, or simply keeping up with a valued patient’s condition on a day-to-day basis, you’ll appreciate the extra lengths we go to keep you involved in your patient’s care while at UNC Health.

UNC Transfer Center

The UNC Transfer Center is a service provided for both internal and external physicians to facilitate the transfer of patients into UNC Hospitals. This service should be used for any type of facility-to-facilty transfer request and can be used for transfers to UNC Hospitals  Emergency Department, Level 1 Trauma Center, Jaycee Burn Center, psychiatric requests, or any other clinical service. This service should not be used for outpatient requests or consultation. Contact the Transfer Center toll-free at 1-800-806-1968.

UNC Hospitals Neurology Physician Referrals

Neurology referrals may be made online on UNC CareLink or by fax. If submitting by fax, please note that neurology has three referral forms:
